Hebrew SeniorLife (HSL) provides enhanced housing with services at affordable housing locations - some of which are our own, and others of which are partner sites. The responsibility of the Wellness Coordinator is to partner with housing staff and community partners to provide a robust program of supports. At its core, enhanced housing with services is a proactive approach wherein resident services staff regularly and actively reach out to each individual resident to engage with them around their health and wellness, identify areas of need/risk and provide intensive, individualized support as needed and desired by the resident.
The Wellness Coordinator works in partnership with housing staff to engage residents in wellness assessments and health education programs, connect residents to needed services, provide a wide variety of case management tasks, ensure that all interventions are documented and tracked, and to develop and strengthen partnerships with community provider organizations as needed. The Wellness Coordinator is part of the housing team working to support residents in living independently and safely for as long as possible.
