Overview:

Sentara Healthcare is seeking a Transplant Coordinator for our Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital. 

Shift: Days M-F

Outpatient clinic is open Monday – Friday between 8am and 4:30pm.  Transplant Coordinator will have some scheduled call and have weekend call about every 11th week.

Position Highlights:

Innovation: 525-bed Level 1 trauma center and tertiary care facility that was first in the region to perform kidney, heart, and pancreas transplants.

Market: Sentara Transplant Center has been around for nearly 40 years and is the only transplant center in Hampton Roads Region. We have performed more than 2,200 transplants.

Nursing Excellence: We were the first hospital in the region to become a Magnet® recognized hospital.

What you will do: 

  • Coordinate care with post-transplant patients.

  • Facilitates clinic flow, transplant evaluation, and patient care management.

  • Provide weekly education sessions to patients and support member.  Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.

  • Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.

  • Transplant Coordinator will manage a case load of patients, remain patient’s point of contact through the transplant process, recovery and at home.

Requirments:

  • BSN required
  • should have 2+ years of acute care experience, good medical - renal knowledge, and understands chronic disease. 
  • BLS required. 
  • Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.
