The Supervisor-Integrated Health and Social Care supervises a team of community health workers, provides direct Community Health Worker services, and assists the community health leader in core community health department functions. A community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The supervisor trains, monitors, and evaluates CHWs according to performance standards. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

Position Details:

Minimum Required Qualifications Education: Bachelor's degree in community health, public health, or other with related coursework Licensure: None Experience: 3 or more years CHW experience or equivalent Certification/Registration: Certified in at least one chronic disease certification program (i.e. DPP) Basic Life Support Other Skills and Knowledge: Strong relationship and collaboration skills Excellent communication skills Ability to work independently Good leadership and team management skills Compassion for poor and underserved members of the community Epic electronic medical record, reporting, dashboards Preferred Qualifications Education: None Licensure: None Experience: None Certification/Registration: Community Health Worker Certification if available Other Skills and Knowledge: Additional language skills as applicable to community
