Social Worker - Learning Disability

Liquid Personnel is recruiting an Adult Social Worker to join its client’s Learning Disability team, based in Rhondda Cynon Taf.

What will your responsibilities be? 

• Safeguard and promote the wellbeing of adults through effective assessment, care planning, and review
• Undertake assessments in line with the Social Services and Wellbeing (Wales) Act 2014
• Identify personal outcomes and develop or review care and support plans as required
• Assess and manage risk in a balanced and proportionate way that promotes independence, choice, and co production
• Build on individual strengths to achieve positive and sustainable outcomes
• Work collaboratively with professionals and external agencies as part of a multi-disciplinary approach
• Maintain accurate and up to date electronic records, including case notes, assessments, and reviews, in line with departmental policies

• Community based role with travel required across the service area
• Hybrid working model with one mandatory office day per week and flexibility for additional office attendance if preferred

Qualifications and Experience: 

To be successful in this role you must have;  

• Qualified Social Worker with relevant registration
• Proven experience within adult services, ideally within learning disability teams
• Strong assessment and review experience
• Ability to work independently and as part of a team
