Job description
At Medical Hill Healthcare, our Social Services Director is a compassionate leader and advocate who directly influences the well-being of our residents. You will create and oversee programs that ensure the emotional and social needs of our residents are met, fostering their independence and dignity every day.
How You Will Make a Difference
  • Empower Residents: Help residents set goals and achieve the highest possible level of self-care and independence.
  • Provide Compassionate Support: Offer emotional support and guidance to residents and their families through difficult times, such as coping with new disabilities or grief.
  • Plan for Success: Lead discharge planning efforts by coordinating with the care team and external agencies to ensure a smooth transition for residents.
  • Connect to Resources: Schedule and coordinate all necessary on-site ancillary services and outpatient appointments.
  • Manage Practical Needs: Oversee personal shopping assistance and the inventory of resident belongings, and manage incident responses for lost or stolen items.
  • Foster Relationships: Build and nurture positive relationships with residents, their families, and the interdisciplinary team through clear and empathetic communication.
What You'll Need to Succeed
  • Experience: A Bachelor's degree in Social Work or a Human Services field with two years of supervised social work experience in a healthcare setting. An MSW is a plus.
  • Qualities: The ability to handle pressure and complex emotional situations with confidentiality and objectivity.
  • Competencies: Strong organizational, planning, and verbal communication skills, with a persistent and proactive approach to problem-solving.
