Job description
Lone Tree Post Acute is seeking a highly skilled and compassionate Speech-Language Pathologist to join our dedicated in-house therapy department. Compensation and benefits

Essential duties and responsibilities
The SLP will be responsible for the evaluation, diagnosis, and treatment of patients with a range of communication, cognitive, and swallowing disorders. Key duties include:
  • Conducting comprehensive, age-appropriate evaluations for adult and geriatric patients.
  • Developing and executing individualized treatment plans in accordance with physician orders.
  • Maintaining thorough and timely documentation, including evaluations, daily notes, and progress reports, to comply with state and payer regulations.
  • Actively participating in patient care conferences and interdisciplinary team meetings.
  • Providing in-service training to facility staff to promote best practices.
  • Instructing patients, families, and nursing staff on maintenance and discharge programs.
  • Supervising and mentoring clinical fellowship year (CFY) candidates or speech-language pathology students.
  • Adhering to the California State Practice Act and facility policies and procedures.
  • Cultivating positive and effective relationships with patients, families, and staff.
Qualifications
  • Education: Master's or Doctoral degree in Speech-Language Pathology from an accredited program.
  • Licensure: Current and unrestricted license as a Speech-Language Pathologist in California, or a Clinical Fellowship Year (CFY) candidate seeking mentorship.
