We’re seeking a compassionate, clinically strong Physician (MD/DO) to serve as a key medical leader within our SNF partnerships — combining hands-on patient care with clinical oversight and interdisciplinary collaboration.

In this role, you’ll:

• Lead and support high-quality care for SNF residents

• Partner with facility leadership, nursing, and care teams

• Manage complex geriatric and post-acute cases

• Drive quality outcomes, compliance, and care coordination

• Support transitions of care, goals-of-care discussions, and operational excellence

What We’re Looking For:

• MD or DO with active unrestricted medical license

• Board Certified/Eligible in Internal Medicine, Family Medicine, Geriatrics, or related specialty

• Experience in SNF, post-acute, geriatrics, or long-term care preferred

• Passion for relationship-centered, patient-focused care

Compensation (from employer):
Pay Range for Posted Region — 220,000 – 330,000 USD
