Are you an experienced Registered Nurse Case Manager / Care Coordinator in Minneapolis, MN looking for a new opportunity with a prestigious healthcare company? Daily Responsibilities:

• Coordinates patient care across the care continuum both pre and post-transplant for patients of all ages.

• Collaborates with MD, interdisciplinary team members, community members, patients, families, and significant others to plan, manage, evaluate care systems and resources.

• Serves as liaison with transplant physicians to implement medical management of condition changes, diagnostic test results, lab results, based on knowledge of patient and patient condition.

• Provides triage and makes patient care decisions. Responds to patient phone calls about condition changes. Directs medical care and evaluation as appropriate.

• Coordinates and identifies the need to include other professional expertise; makes arrangements, both local and national, to include care providers in a multi-disciplinary treatment plan.

• Provides direct patient assessment and evaluation: obtains patient history, provides physician with information on patient status


Hours for this Position: : Monday-Friday – Day shift

Pay Rate: 30.00-40.00 negotiable based on relevant experience

• 3 years Acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg caring for complex pancreas patients or other relevant nursing experience-Required
