PROMISE TO HOPE-CARE MGR-BSN

The Promise to Hope Care Manager functions in a multi-faceted role that demonstrates professionalism and promotes excellence in the practice of Substance Abuse. The Promise to Hope Care Manager is responsible for maintaining high standards as described by the scope of care for the area of practice and demonstrates appropriate actions based on sound scientific research and evidence-based principles. The Promise to Hope Care Manager also coordinates care delivered by an interdisciplinary team. The Promise to Hope Care Manager is accountable for compliance with applicable regulatory standards and institutional policies and procedures. The Promise to Hope Care Manager provides physical, emotional and spiritual support that directly/indirectly impacts the patient outcomes and care provided to patients/families/significant others receiving care from this program. Patients served will be those experiencing substance misuse concerns. The Promise to Hope Care Manager will work collaboratively with relevant external agencies and prenatal services to ensure extensive knowledge of available resources as well as a defined process to identify clients that will benefit from the Promise to Hope Program.
