At Lewis Park Post Acute, we are dedicated to providing exceptional care in both long-term and short-term post-acute settings. As a Registered Nurse (RN), you’ll lead and coordinate nursing care for residents recovering from illness or injury as well as those requiring long-term support. You’ll work as part of an interdisciplinary team, guiding care plans, supporting staff, and ensuring our residents receive the compassionate and professional care they deserve.
Key Responsibilities:
- Provide and coordinate nursing care based on comprehensive assessments and individualized care plans.
- Administer medications and treatments safely and accurately.
- Monitor and assess residents’ conditions; respond to changes and report to physicians and family members as appropriate.
- Serve as a leader and clinical resource to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).
- Oversee admissions, discharges, and transitions of care with a focus on continuity and quality.
- Maintain accurate documentation in compliance with regulatory standards and facility policies.
- Participate in care plan meetings, quality improvement initiatives, and ongoing training.
- Ensure infection control, safety, and emergency protocols are followed.
- Advocate for residents’ needs, dignity, and rights.
