Apply to this jobEvaluate patients within 24 hours of physician referral and document any delays; develop and implement treatment plans with physician approval.
Provide direct patient care in accordance with treatment plans; supervise and guide Physical Therapy Assistants and Restorative Aides.
Participate in care conferences, utilization reviews, and in-service training for staff; assist in discharge planning and caregiver training.
Maintain detailed and timely documentation, including evaluations, treatment notes, progress notes, and recertifications per state and payer requirements.
Communicate effectively with supervisors, healthcare team members, patients, and families regarding patient progress and care plans.
Recommend, order, and facilitate necessary durable medical equipment (e.g., walkers, braces) to support patient mobility and independence.
Ensure all therapy equipment is properly maintained; report and address any equipment issues promptly.
Maintain current Physical Therapist licensure and meet ongoing education requirements per state regulations; proficiency with Casamba and Point Click Care is preferred.
Perform physical tasks regularly, including lifting over 50 lbs, standing, walking, pushing/pulling, and occasional stooping or kneeling.
Foster positive, effective relationships with patients, families, staff, and colleagues; uphold all policies regarding safety, confidentiality, and professional standards.
