Status: Full-Time / Part-Time / Per Diem

Westgate Gardens Care Center is seeking a dedicated and compassionate Physical Therapist to join our skilled rehabilitation team. If you're passionate about improving the lives of others through exceptional therapy services, we’d love to meet you!

Responsibilities:

• Evaluate patients within 24 hours of physician referral and document accordingly.

• Develop and implement effective treatment plans in coordination with referring physicians.

• Provide direct patient care based on approved therapy plans.

• Supervise Physical Therapy Assistants and assist in training Restorative Aides.

• Participate in care conferences, utilization reviews, and interdisciplinary meetings.

• Maintain thorough documentation: evaluations, daily notes, progress reports, and recertifications.

• Instruct families and caregivers in maintenance programs for discharge planning.

• Recommend and coordinate durable medical equipment needs.

• Report and help maintain therapy equipment.

Qualifications:

• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy.

• Active PT license in the State of California in good standing.

• Skilled in Casamba, PointClickCare, or other EMR systems preferred.

• Strong interpersonal, organizational, and communication skills.

• Ability to lift up to 100 lbs with assistance and perform physical job duties.
