Physical Therapist (PT) – Full-Time

Victorian Post Acute

Compensation: Starting at $60-$65 hour (DOE)
Schedule: Full-Time, Part Time, PRN - Flexible Scheduling Options
Program: In-House Therapy Team

Key Responsibilities

  • Evaluate patients and develop individualized treatment plans.

  • Provide skilled physical therapy services in accordance with physician orders and established care plans.

  • Supervise Physical Therapist Assistants and ensure compliance with state practice regulations.

  • Collaborate with interdisciplinary team members to coordinate and optimize patient care.

  • Participate in care conferences, rehabilitation meetings, and utilization reviews as needed.

  • Maintain timely and accurate documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries.

  • Educate patients, families, and caregivers on treatment goals, home exercise programs, and safe mobility techniques.

  • Assist with discharge planning and recommendations for continued care.

  • Recommend and facilitate acquisition of durable medical equipment as appropriate.

  • Monitor and report equipment maintenance needs to ensure a safe treatment environment.

Qualifications

  • Current Physical Therapist license in good standing.

  • Strong communication and interpersonal skills.

  • Commitment to providing exceptional patient-centered care.
