Job Title: Physical Therapist (PT)
Location: Gateway Post Acute – Porterville, CA
Starting Pay: $60/hour
Schedule: Full-Time or Part-Time Opportunities Available

Located in Porterville, CA, our skilled nursing facility has been providing compassionate, high-quality care for over 40 years. We’re currently seeking a compassionate and motivated Physical Therapist to join our therapy department.

What You'll Do:

• Evaluate and assess residents’ physical conditions

• Develop and implement individualized treatment plans

• Provide therapeutic interventions, including joint mobilization, therapeutic exercises, and more

• Monitor and document patient progress, adjusting treatment plans as needed

• Educate residents and families on mobility techniques and injury prevention

• Collaborate with interdisciplinary healthcare team to ensure quality patient outcomes

• Conduct in-services and training sessions for facility staff

• Maintain accurate, timely documentation in accordance with regulations and company policy

What We're Looking For:

• Valid California Physical Therapist License in good standing

• Knowledge of anatomy, physiology, and therapeutic techniques

• Strong communication and interpersonal skills

• Proficient in documenting services; familiarity with PointClickCare and/or Casamba is a plus

• Skilled nursing facility experience preferred, but not required
