Physical Therapist, Part-Time, Acute Care | 1:1 Patient Care | Bronson South Haven & Bronson LakeView | $10,000 Sign-On BonusJoin Bronson's rehabilitation team and make an immediate impact on patients' recovery journeys through 1:1 patient care in the acute care setting. Serving both Bronson South Haven and Bronson LakeView, you'll evaluate and treat hospitalized patients experiencing illness, injury, surgery, or functional decline, helping them regain mobility, maximize independence, and safely transition to the next level of care.
As a valued member of an interdisciplinary team, you'll collaborate closely with physicians, nurses, case managers, and rehabilitation professionals to deliver exceptional patient-centered care while supporting positive clinical outcomes.

Key Responsibilities
Clinical Care
- Perform comprehensive physical therapy evaluations for hospitalized patients.
- Assess patient mobility, strength, balance, endurance, functional limitations, and rehabilitation needs.
- Develop and implement individualized treatment plans based on physician referrals, clinical findings, and patient goals.
- Provide direct one-on-one patient care using evidence-based therapeutic interventions.
- Establish measurable goals, monitor progress, and modify treatment plans as needed.
- Collaborate with physicians, nurses, case managers, and rehabilitation professionals to support coordinated care and discharge planning.
- Educate patients and families regarding treatment plans, mobility strategies, home exercise programs, and post-discharge recommendations.
- Participate in interdisciplinary rounds and care planning discussions.
- Serve as a resource to team members and contribute to departmental initiatives and process improvement activities.
- May provide coverage across rehabilitation service locations as needed.
Documentation & Administrative Responsibilities
- Complete timely, accurate, and compliant documentation in the electronic health record.
- Maintain accurate records of evaluations, treatments, patient progress, and outcomes.
- Communicate documentation or workflow barriers to leadership in a timely manner.
- Participate in departmental meetings, committees, and quality improvement initiatives.
Leadership & Mentorship
- Demonstrate effective time management and organizational skills.
- Supervise Physical Therapist Assistants (PTAs), rehabilitation support staff, students, and volunteers as assigned.
- Foster a collaborative environment that supports learning and clinical excellence.
Qualifications
Required
- Graduate of an accredited Physical Therapy program.
- Current Michigan Physical Therapist license in good standing.
- BLS certification within three months of hire.
- Strong communication, interpersonal, and patient education skills.
- Ability to work effectively as part of an interdisciplinary team.
- Proficiency with electronic health records and computer systems.
Employees providing direct patient care must demonstrate competencies appropriate to the populations served.
