Job Duties:

• Develop and implement personalized physical therapy treatment plans for residents.
• Assist with the new sensory program, focusing on resident engagement and sensory stimulation.
• Collaborate with interdisciplinary teams to ensure comprehensive resident care.
• Conduct evaluations to determine therapy needs and document progress.
• Educate residents and their families on at-home exercises and wellness strategies.
• Maintain accurate records and comply with all facility policies and procedures.

Qualifications:

• Current state licensure as a Physical Therapist.
• Strong communication and collaboration skills.
• Passion for working with residents in a post-acute care setting.
• Experience with sensory programs is a plus!
