OVERVIEW: THIS IS NOT YOUR AVERAGE PT JOB.

This is an opportunity to evolve—as a clinician, as a professional, and as a leader in modern physical therapy.

Evolve Physical Therapy, powered by Rise Therapy Partners, is one of Brooklyn’s most trusted and preferred physical therapy practices. We’re growing quickly because we do things differently: we value clinical excellence, autonomy, and evidence-based care, and we’ve built an environment where therapists can thrive without burning out.

If you’re a Physical Therapist who wants to transform lives, sharpen your clinical thinking, and be part of a team that’s actively pushing the status quo of physical therapy in New York—you’ll feel at home here.

WHY EVOLVE?

At Evolve, we believe great outcomes come from empowered clinicians. That means:

Evidence-Based Practice First: Clinical decision-making rooted in current research, outcome tracking, and thoughtful progression—not cookie-cutter protocols.

Collaborative, Growth-Oriented Culture: You’ll work alongside motivated clinicians who love to learn, challenge assumptions, and support one another in a family-friendly vibe.

Real Career Growth: Whether your goals are advanced clinical expertise, mentorship, leadership, or long-term career development—we build pathways, not ceilings.

Deeply Embedded in the Brooklyn Community: We’re proud to be the go-to PT practice for Brooklynites who expect high-quality, personalized care.

WHAT YOU’LL DO (RESPONSIBILITIES)

⦿ Conduct Initial Evaluations, generate therapy diagnoses, and implement comprehensive Plans of Care.
⦿ Deliver high-quality 1:1 patient care with a focus on functional outcomes and long-term success.
⦿ Evaluate, diagnose, and treat a diverse orthopedic and movement-based caseload.
⦿ Maintain timely and accurate clinical notes.
⦿ Manage your own caseload and see patients throughout their treatment journey.
⦿ Participate in mentorship, case discussions, and professional development initiatives.

WHO WE’RE LOOKING FOR (REQUIREMENTS)

⦿ Graduate of an accredited American Physical Therapy Association school (Master's preferred).
⦿ Active New York State Physical Therapy License.
⦿ CPR Certification (Preferred).
⦿ Strong customer service skills, high energy, and a positive attitude.
⦿ Socially and emotionally intelligent, connecting with diverse patients.
⦿ New grads and experienced clinicians encouraged to apply.

WHAT WE OFFER (BENEFITS & PERKS)

⦿ Financial & Future: Competitive salary ($78.5k-$96k), quarterly bonuses based on profitability, 401(k) with 3% matching, and retirement planning.
⦿ Health & Wellness: Super affordable health plan, dental insurance, HSA/FSA options.
