Salary: $85K - $100K, plus per visit stipend, depending on experience and hire status (PRN or Full Time)

Custom Physical Therapy is an outpatient physical therapy practice with four locations treating various physical conditions across Reno and Sparks. Our number one priority is our patients. We provide personalized treatment with hands-on, compassionate care. We perform comprehensive evaluations and encourage patient input for treatment planning and goal setting. Responsibilities

• Assesses patients and develops and initiates treatment plans based on the evaluation results.
• Review the patient's condition and medical records to determine the physical therapy treatment required.
• Provides direct patient care by established protocols and patient care guidelines.
• Documents findings, progress, and instructions to patients and caregivers.
• Holds responsibility for patient care performed by assistants and rehabilitation technicians.
• Completes a written plan of care, develops goals based on evaluation findings, and develops a comprehensive program to attain goals.
• Implements physical therapy treatment program and provides essential physical therapy treatments.
• Communicates effectively with patients/caregivers and professional colleagues, including physicians and insurance companies.
• Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports.

• Graduate from an accredited college with an APTA curriculum
• One or more years of outpatient orthopedic experience within a private clinic or hospital preferred
• Currently registered and licensed in the state of Nevada and CPR certified
• Outgoing and energetic personality
• Experience in exercise and manual-based skills, McKenzie skilled, and sports rehabilitation

The anticipated base salary range for this position is $45 - 50.00 an hour.  Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for discretionary incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
