Job Title: Certified Occupational Therapist (OT) – PRN
Location: Gateway Post Acute – Porterville, CA
Starting Wage: $60/hour
Schedule: PRN (As Needed)

Gateway Post Acute, located in the heart of Central California near the scenic Sierra Nevada foothills, is a premier skilled nursing facility known for its compassionate care, clean environment, and supportive team culture. We are currently seeking a Certified Occupational Therapist (OT) to join our therapy team on a PRN basis.

About the Role:

The Occupational Therapist provides hands-on rehab care to patients referred by physicians. This role focuses on helping patients regain independence in daily living and functional activities. You will work closely with interdisciplinary teams, patients, and families to achieve positive care outcomes in compliance with all regulations and facility standards.

Key Responsibilities:

• Evaluate and treat patients in accordance with physician referrals and care plans

• Document assessments, progress notes, and outcomes in EMR systems

• Educate patients and families on adaptive techniques and therapeutic goals

• Collaborate with nursing and rehab staff to develop individualized plans

• Maintain professional relationships with staff, families, and vendors

• Ensure patient safety, rights, and confidentiality at all times

• Participate in community health efforts and facility meetings as needed

Qualifications:

• Current California OT license (in good standing)

• CPR certification (or obtained during orientation)

• 1 year of OT experience in post-acute or skilled nursing preferred

• Proficient in documentation and therapy software (e.g., PointClickCare, Casamba)

• Strong communication, clinical, and organizational skills

• Knowledge of post-acute regulatory guidelines and patient rights
