Vineyard Post Acute is seeking Occupational Therapists (OTs) to join our dedicated rehabilitation team. Job Responsibilities

• Evaluate and develop individualized treatment plans for residents
• Implement evidence-based therapy interventions to improve daily living skills
• Work collaboratively with an interdisciplinary team to enhance resident outcomes
• Educate residents and families on adaptive techniques and rehabilitation goals
• Maintain accurate documentation in compliance with regulatory standards

Qualifications

• Active Occupational Therapist (OT) license in California
• Passion for helping patients regain function and independence
• Strong teamwork and communication skills
• SNF experience preferred, but new graduates are encouraged to apply
