Nursing Home Clinical Supervisor – TBS (Nursing Home Program)
Location: Partner Nursing Homes – Cleveland, Ohio
Position Type: Full-Time

We are a growing behavioral health organization dedicated to delivering compassionate, high-quality mental health and therapeutic services to vulnerable populations. We are currently seeking an experienced and independently licensed Nursing Home Clinical Supervisor – TBS (LPCC-S or LISW-S) to provide clinical leadership and oversight for our Therapeutic Behavioral Services (TBS) programming across multiple partner nursing homes and long-term care facilities in the Cleveland, Ohio area. This pivotal role ensures the delivery of compliant, effective, and resident-focused behavioral health services while mentoring and developing a multidisciplinary team of behavioral health providers, including group facilitators and therapists.

Key Responsibilities

Supervision & Clinical Oversight

  • Provide regular individual and group clinical supervision to qualified behavioral health staff, including LPCs, LSWs, Qualified Mental Health Specialists (QMHS), and Chemical Dependency Counselors (CDCAs).
  • Ensure all clinical documentation meets or exceeds Medicaid, state, agency, and professional ethical standards.
  • Mentor and support staff development through ongoing training, constructive performance feedback, and professional growth opportunities.
  • Guide the team in delivering evidence-based, trauma-informed, and person-centered clinical interventions.
  • Conduct case consultations, support risk assessments, and assist staff with crisis intervention and de-escalation.
  • Lead initial onboarding and continuous training for group facilitators and behavioral health team members.
  • Identify training needs and facilitate skill-building initiatives to enhance service quality and team competence.
  • Promote the consistent implementation of person-centered care approaches tailored to the unique needs of geriatric and medically complex residents.

Program Management & Collaboration

  • Oversee the successful implementation and ongoing delivery of TBS group programming in alignment with individualized treatment plans and all regulatory requirements.
  • Collaborate closely with nursing facility staff, interdisciplinary care teams, and internal leadership to ensure coordinated, integrated service delivery.
  • Participate in program evaluation, quality assurance activities, and clinical audits to maintain high standards of care.
  • Attend and contribute to team meetings, internal leadership discussions, and regular communications with partner nursing facilities.

Direct Services & Coverage

  • Provide direct clinical services, including assessments, treatment planning, and group facilitation, to ensure service continuity and address emergent resident needs.
  • Offer clinical consultation to staff and facility teams regarding resident engagement, behavioral concerns, and mental health support.
  • Ensure all treatment plans and services remain clinically appropriate, medically necessary, and fully compliant with Medicaid and organizational standards.

Qualifications

Required

  • Active and unrestricted Ohio licensure as an LPCC-S or LISW-S (LICDC-S-CS will also be considered).
  • Minimum of 2 years of post-independently licensed clinical experience, including supervisory designation.
  • Demonstrated experience delivering Therapeutic Behavioral Services (TBS), Intensive Outpatient Programming (IOP), or similar structured behavioral health services.
  • Strong background working with geriatric populations and/or medically complex individuals in long-term care or similar settings.
  • Solid understanding of Medicaid billing, documentation requirements, and compliance standards.
  • Excellent interpersonal, organizational, and written/verbal communication skills.
  • Proven ability to work independently while traveling between multiple off-site partner facilities.
  • Reliable transportation and willingness to travel within the Cleveland area.

Preferred

  • Prior experience in long-term care or skilled nursing facility environments.
  • Familiarity with electronic health record (EHR) systems (training provided).
  • Certification or specialized training in trauma-informed care (or willingness to obtain within the first 90 days).
  • Flexibility and adaptability to varying facility cultures, team dynamics, and daily operational needs.

Compensation & Benefits

This is a full-time position with a standard schedule of 9:00 AM – 5:00 PM. Compensation is competitive and ranges from $60,000 to $90,000 annually, depending on experience and qualifications. Additional perks include:
