  • Guide the team in implementing evidence-based, trauma-informed interventions and person-centered care approaches.

  • Lead initial onboarding and continuous training for group facilitators and other behavioral health team members.

  • Attend and contribute to team meetings, leadership discussions, and partner facility communications.

Direct Services & Coverage

  • Provide direct clinical services, including assessments, treatment planning, and group facilitation, to ensure service continuity and address emergent needs.
  • Offer consultation to facility staff regarding resident engagement, behavioral concerns, and mental health needs.
  • Support the development and review of clinically appropriate treatment plans.
  • Maintain thorough and timely documentation that complies with Medicaid, state, and organizational standards.

Qualifications:

Required:

  • Active and unrestricted Ohio licensure as an LPCC-S, LISW-S, or LICDC-CS (supervisory designation required).
  • Minimum of 2 years of post-independently licensed clinical experience.
  • Demonstrated experience with Therapeutic Behavioral Services (TBS), IOP, or similar behavioral health group programming.
  • Strong background working with geriatric populations and/or medically complex individuals.
  • Solid understanding of Medicaid billing and documentation compliance requirements.
  • Excellent interpersonal, organizational, and written communication skills.
  • Ability to work independently and travel across multiple off-site partner facilities in the Cleveland area.
  • Reliable transportation and a valid driver’s license.

Preferred:

  • Prior experience in long-term care or skilled nursing facility settings.
  • Familiarity with electronic health record (EHR) systems (training provided).
  • Certification or training in trauma-informed care (or willingness to obtain within 90 days).
  • Licensure as LSW or LISW strongly preferred, or equivalent social work licensure based on jurisdiction.
  • Flexibility and adaptability to varying site conditions and team dynamics.

Compensation & Benefits:

  • 18 days of PTO plus 6 paid holidays and a paid birthday day off.
  • Referral bonuses.
  • CEU support and professional development opportunities.
  • Potential for growth into broader clinical leadership roles.
