The Program Coordinator – Mobile Health Services (MHS) works closely with the Administrative Site Director (ASD) to provide coordination of Open Door Community Health Centers’ (ODCHC) Mobile Health Services for patients experiencing homelessness; coordination duties include mobile health van upkeep, host site coordination and logistics, site setup and take down, and daily patient intake. The MHS Program Coordinator is responsible for driving the Mobile Medical Van to scheduled services areas and outlying clinic locations. While the Mobile Medical Van is stationed at the scheduled service areas the MH Program Coordinator works to greet and register patients and maintain a safe and welcoming environment.

Compensation Range:

$30.00-$34.84

Hmong and Spanish speakers who successfully pass a language exam will receive an additional .75 hourly differential to their wage.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Drives the mobile health van from Corporate Services to and from outlying services sites.
  • Keeps the mobile unit in good working order, either making repairs, or arranging for repairs and servicing when needed.
  • Keeps gas and water tanks full and waste tanks empty.
  • Keeps receipts, transportation logs and vehicle records. Coordinates with Corporate Services in keeping vehicle registration up to date.
  • Works with the ASD and Registered Nurse Clinic Manager (RNCM) to ensure the mobile health van is fully compliant with ODCHC, State, and Federal requirements as it pertains to routine site reviews and administrative audits.
  • Performs housekeeping duties to keep the mobile health van clean and organized.
  • Acts as a mobile receptionist for patients visiting the mobile health van.
  • Reviews and updates patient demographic, billing, and visit information in Electronic Medical Records (EMR) at each patient visit.
  • Checks all patients for Medi-Cal and County Medical Services Program (CMSP) eligibility at time of service.
  • Obtains copies of insurance cards from patients and screens uninsured patients for eligibility for state programs and performs onsite enrollment or refers patient to the Social Work Case Manager.
  • Stays up-to-date on the requirements and procedures for enrollment in state and county programs. 
  • Keeps an organized and adequate supply of current versions of all patient registration, billing, and enrollment forms in both English and Spanish.
  • Accurately screens out intoxicated patients and warns clinic staff if a patient appears potentially violent.
  • Alerts clinical staff if a patient may need to be seen immediately due to urgent conditions.
  • Maintains communication with ODCHC care team and enabling services as needed to foster integrated whole person care for each patient.
  • Engages in direct patient outreach to ensure the highest level of access and coordination of care for those in our community most affected by the social determinates of health.
  • Works as liaison between mobile health van staff and host site to maintain a safe and comfortable environment for all in the waiting area.
  • Coordinates and facilitates communication with community partners-in-care to resolve challenges, including ODCHC site leadership as needed.
  • Participates in appropriate staff meetings.
  • Performs other duties and responsibilities as designated by supervisor.

QUALIFICATIONS AND EXPECTATIONS:

  • Ability to effectively represent ODCHC’s interests in the community and maintain effective working relationships among co-workers, public, private, and professional groups.
  • Strong computer skills with specific aptitude in Microsoft Office Suite and/or Electronic Medical Records (EMR).
  • Experience driving a large recreational vehicle with emphasis on safety. 
  • Adheres to ODCHC’s policies and procedures.

EDUCATION AND EXPEREINCE:

  • High school diploma or GED.
  • Six months of experience in a similar role; or an equivalent combination of education and experience.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Valid California driver’s license.
  • Current CPR certification.

SUPERVISORY RESPONSIBILITIES: None.

SUPERVISION AND SUPPORT: The Program Coordinator – Mobile Health Services reports directly to the Administrative Site Director and is an integral member of the Mobile Health Services team.
