LVN (Part-Time) - Sherwood Oaks Post Acute

PM Shift (3pm-11pm) and NOC (11pm-7am) shift available part-time on the weekends

Provide and oversee direct nursing care to residents in accordance with care plans and ensure compliance with federal, state, and facility standards.

Supervise and support nursing assistants, including assigning tasks, conducting evaluations, and ensuring adherence to policies and procedures.

Administer medications and treatments as prescribed, monitor for adverse effects, and maintain accurate narcotic records and documentation.

Complete thorough and timely charting, including progress notes, physician’s orders, incident reports, and discharge plans.

Coordinate care plans and assessments, ensuring nursing services align with residents’ goals and that care is consistently documented and reviewed.

Participate in facility inspections and quality assurance programs, ensuring the nursing department meets all regulatory and internal standards.

Lead and participate in staff development, including orientation, training, and continuing education for nursing personnel.

Ensure resident safety and infection control, including proper use of PPE, hygiene procedures, and maintaining clean, safe environments.

Communicate with residents, families, and interdisciplinary teams, providing updates on care plans, responding to concerns, and ensuring satisfaction with services.

Respond to emergencies and incidents, including falls, changes in condition, deaths, and occupational exposures, following facility procedures and reporting as required.
