Home Health LVN Position Summary: 

Work as a vital member of the Home Health care team, providing hands-on support to patients while utilizing AI-supported technology to simplify your documentation process. Spend more time delivering care and less time on paperwork. 

Patient-Centered Care:Spend more time providing hands-on, compassionate care to patients. 

Reduced Documentation Burden:Use AI-supported tools to streamline clinical notes and regain personal time. 

Work-Life Balance:Cut down on after-hoursdocumentation, allowing more time for family and personal priorities. 

Enhanced Efficiency:Experience an easier transition to technology-driven workflows that simplify compliance with care standards. 

Key Responsibilities: 

  • Assist with medication management, wound care, and health monitoring. 
  • Use AI systems to assist in completing clear and accurate notes during visits. 
  • Focus on patient interaction and quality of care by reducing administrative burden. 

Education and/or Experience 

  • Graduate of an accredited Licensed Practical/Vocational Nurse program. 
  • Work Experience: Minimum 1 year as an LVN (6 months home care preferred).

  • Certifications/Licenses: California Licensed Vocational Nurse required upon hire; Infusion and Venipuncture Certification required; National Provider BLS – American Heart Association within 30 days of hire. Valid driver’s license and auto insurance required upon request.

Certifications, Licenses, and Registrations 

  • Active and unencumbered Licensed Vocational/Practical nurse license in state(s) of employment required. 
  • Preferred within 30 days of hire: National Provider BLS - American Heart Association  

Pay Range: $34.21-$35.38
