The Inpatient Case Manager is responsible for the assessment, treatment planning, intervention, monitoring, evaluation and documentation on identified patients during an acute care episode. ESSENTIAL RESPONSIBILITIES and DUTIES:

  • Know and follow Employee Handbook and company policies and procedures.
  • Know and adhere to contents of Policy and Procedures Manual, Communication Book, Safety Manual, MSDS.
  • Follow procedures for completing medical documentation and forms.
  • Adhere to the Nursing Standards of Excellence.
  • Communicate verbally and in writing through proper channels.
  • Maintain patient/employee confidentiality.
  • Adhere to all HIPAA, company privacy and confidentiality policies.
  • Use time keeping system appropriately.
  • Participate in required meetings.
  • Follow dress code.
  • Notify supervisor through time keeping system when requesting paid time off.
  • Be courteous.
  • Be flexible and adaptable.
  • Promote departmental and organizational goals.
  • Know and follow applicable safety standards.
  • Work amicably with hospital staff to coordinate the care of HSMG patients.
  • Conduct rounds at the facilities as required.
  • Utilize computers to access patient records.
  • Meet with patients and family to coordinate care during and after discharge.
  • Acquire approval from HSMG Medical Director for any and all transfers including higher level of care, and facilitating those transfers.
  • Charting and utilization review of those patients currently admitted.
  • Perform other duties as assigned consistent with education and experience.

PHYSICAL DEMANDS:

  • Sitting - approximately 75%.
  • Standing - approximately 10%.
  • Walking - approximately 10%.
  • Lifting - 0 to 10 lbs: charts, etc. - frequency - approximately 10%.
  • Bending, stooping and squatting - frequency - approximately 10%.
  • Reaching above head to 75 degrees - frequency - approximately 10%.
  • Hand/grip dexterity - phone work, writing, data entry, etc. - frequency - approximately 65%.
  • Climbing stairs - typically one flight - frequency - approximately 5%.
  • Must be able to drive a car.

The pay range for this position at commencement of employment is expected to be between $33.00 and $36.00/hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Details of participation in these benefit plans will be provided if an employee receives an offer of employment.
