Job Description:

Develops the plan of care, collaborates with the physician in development of the plan of treatment, and supervises services provided in the client’s home by licensed practical and registered nurses.

Education:

  • Required: Graduate of an accredited nursing program

Experience:

  • Required: Minimum of three (3) years of experience (one (1) year must be in high-acuity pediatric care in a hospital, home care agency, or residential setting
  • Preferred: One (1) year of home care experience
  • Preferred: Supervisory experience

Skills:

  • Ability to function independently, organize and prioritize responsibilities efficiently and demonstrate good nursing judgement
  • Ability to work well under stress and adapt to changing conditions
  • Ability to work in various home environments
  • Ability to deal effectively and courteously with clients, families, coworkers, physicians, and community resources and exhibit empathetic attitude toward the sick and elderly
  • Ability to effectively communicate verbally and in writing
  • Demonstrates computer and general office machine skills

Licensure/Certification:

  • Required: LPN License active in the state of Missouri
  • Required: Active Driver’s License
