LPN Intake Coordinator

Columbia River Home Health is seeking an LPN Intake Coordinator to manage referrals, coordinate admissions, verify insurance, and serve as a key liaison between patients, families, referral partners, and our clinical team.

What You'll Do

  • Manage incoming referrals and coordinate admissions
  • Review referrals for clinical appropriateness
  • Communicate with patients, families, physicians, hospitals, and referral sources
  • Verify insurance benefits and authorizations
  • Coordinate seamless transitions of care
  • Maintain accurate documentation and patient records
  • Build and maintain strong relationships with referral partners

Qualifications

  • Current Washington State LPN license
  • Healthcare experience required; home health, hospice, intake, referrals, or care coordination experience preferred
  • Strong communication, organization, and customer service skills
  • Ability to multitask and work independently in a fast-paced environment
  • EMR and computer proficiency
