Start Date: ASAP
Duration: 6 Months (option to extend)
Hourly Rate: £31-36

Job Overview

Pulse is currently seeking two experienced Clinical Pharmacists (Band 7 / Band 8a) to join a busy acute hospital team in Croydon. This is an excellent opportunity for hospital pharmacists with strong ward-based experience to contribute to the delivery of safe, effective, and patient-centred pharmaceutical care within a dynamic NHS environment.

The successful candidates will provide clinical pharmacy services across hospital wards, supporting medicines optimisation and ensuring the safe prescribing and use of medicines throughout the patient journey.

 

Key Responsibilities

  • Undertake comprehensive medication histories and medicines reconciliation on admission
  • Provide ward-based clinical pharmacy services as part of the multidisciplinary team
  • Conduct clinical screening of inpatient prescriptions and medication charts
  • Review and screen TTAs (To Take Away medications) prior to discharge
  • Identify, resolve, and prevent medication-related issues to optimise patient outcomes
  • Support medicines optimisation initiatives and clinical governance activities
  • Provide expert pharmaceutical advice to medical, nursing, and allied health professionals
  • Ensure compliance with Trust policies, national guidelines, and medicines legislation
  • Utilise Electronic Prescribing and Medicines Administration (EPMA) systems effectively within daily practice

 

Qualifications and Experience

  • Registered Pharmacist with the General Pharmaceutical Council (GPhC)
  • Previous NHS hospital pharmacy experience is essential
  • Significant experience working in a ward-based clinical pharmacy role
  • Proven experience undertaking:
    • Drug histories
    • Medicines reconciliation
    • Clinical screening
    • TTA screening
  • Experience using EPMA systems is essential
  • Experience with Cerner EPMA is highly desirable
  • Excellent clinical knowledge, communication, and organisational skills
  • Ability to work effectively within a multidisciplinary team
