A little more about our center:

Crestwood Terrace is an Intermediate Care Facility, providing care to the mentally ill/behavior health population (ages 21 and up).  Located in a quiet neighborhood, we are one of 3 medical centers on our street.  What makes us unique:

The purpose of this position to is to assist the Activities team in creating a positive, engaging, and supportive environment for adults receiving long-term care and mental health services. Working alongside the Activities team, this position assists with planning and leading recreational, social, educational, and therapeutic activities that encourage participation, recovery, independence, and meaningful connections.

  • Assist in planning and leading fun, engaging and therapeutic activities that encourage guest participation, socialization and personal growth. 
  • Work collaboratively with the Activities team to support each resident's individual goals and recovery journey and communicate any concerns to the Activity Director. 
  • Monitor participation, document progress, and help evaluate the effectiveness of programs. 
  • Bring fresh ideas to create engaging events, seasonal celebrations, and meaningful daily activities.
  • Build positive relationships with residents while promoting social interaction, confidence, and meaningful engagement. 
  • Create a warm, welcoming, and inclusive environment where every reskdent feels respected, valued, and encouraged. 
  • Maintain a safe, organized, and positive activity environment. 
  • Perform other duties or responsibilities as assigned.
  • High school diploma or equivalent required.
  • Experience working in a long-term care or post-acute care setting is preferred, but not required.
  • Basic computer knowledge required. 
  • Ability to read, write, speak and understand English. 
  • Must be able to push, pull, move and/or lift a minimum of twenty-five pounds 
  • Ability to stand for one-hour intervals for up to 75% of the workday 
  • Must successfully pass a pre-employment physical examination

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
