Based in the heart of downtown Lowell, Lowell Community Health Center is currently seeking a Full-Time Licensed Behavioral Health Clinician.

Clinician may be responsible for completing outpatient screenings/assessing need for BHS services through brief evaluation. Case assignment follow/up will be required, also working with the care coordinator and front desk staff for appropriate fit. Clinician will also be assigned a caseload and will follow BHS productivity requirements. Clinician may be assigned work within the various medical departments to offer integrative services for Behavioral Health/Primary Care.

 
ESSENTIAL DUTIES AND RESPONSIBILITIES: (Other duties may be assigned)

  • Work in tandem with the BHS Care Coordinator to schedule screenings/assessment appointments for individuals referred for Outpatient Behavioral Health Appointments.
  • Complete the Orientation Packet and other required initial case paperwork.
  • Complete the TB, HIV, Tobacco Screens and Risk Assessments.
  • Complete the Basis 24 and/or CANS as clinically indicated.
  • Complete the BPS Assessment for all patients assigned for treatment, and other relevant documentation.
  • Collaboratively develop a treatment plan, with patient.
  • Present all completed intakes at the weekly BHS multi-disciplinary treatment team.
  • Document all treatment encounters and outreach contact in eCW.
  • Maintain statistics for completed intakes, and individuals screened out to external agencies and organizations and track individuals who failed the intake process.
  • Complete a discharge note and/or summary as clinically indicated.
  • Provide appropriate supported referrals for individuals screened out of BHS outpatient care.
  • In-House Consultations and On-Call Rotation may also be required.
  • Develop and facilitate one outpatient treatment group and complete all required clinical documentation.
  • Attend community meetings as assigned.
  • Participate in the BHS multi-disciplinary treatment team.
  • Participate in individual clinical supervision as indicated.
  • Maintain a 65% direct care rate.

MA in Counseling, Psychology, Social Work, Family Therapy or related field. Bi-Lingual in Spanish/English preferred.
LCSW, (LICSW eligible within 2 years of hire), LICSW, LMHC or LMFT required.
