Hospice Social Worker

Overview

Hospice Social Worker provides a wide variety of direct and indirect social work services to patients/caregivers/families in accordance with the individualized plan of care, as prescribed by the physician.  The Hospice Social Worker actively participates in coordination of all aspects of the patient’s care, in accordance with current professional standards and practice.  The Social Worker participates in ongoing interdisciplinary comprehensive assessment, develops and evaluates the Plan of Care, contributes to patient and family counseling and education and participates in the QAPI program and hospice sponsored in-service training. 

 

Schedule: Monday-Friday: 8:00am-5:00pm including responsiblity for on call rotation coverage.

 

Location:  Falls Church and Arlington, VA

 

COVID-19 vaccine required to be completed upon start

Responsibilities

The Hospice Social Worker is an active member of the Capital Caring Health Interdisciplinary Team.  

Qualifications

Experience Requirements

Must have two years of Master’s level experience (preferably in an interdisciplinary health care setting)

 

Education Requirements

Must have a Master's degree in Social Work from a school accredited by the Council on Social Work Education.

 

Licensing Requirements

Must be licensed or license eligible in compliance with jurisdiction of practice (LCSW, LICSW).

 

Driver's License

Must have a driver's license in good standing and access to a vehicle during working hours.
