Essential Functions:

  • Manages primary caseload of Hospice patients per physician's instructions and in compliance with regulations.
  • Implements and updates care plans to address patient's nursing needs.
  • Educates and supports patients, family, and staff on nursing care and patient needs.
  • Collaborates with the Interdisciplinary Team (IDT) to meet all patient and family care requirements.
  • Supervises Licensed Practical Nurses and Hospice Aides.

Requirements:

  • Certified Hospice and Palliative Nurse (CHPN), preferred.
  • 2+ years' experience in hospice or palliative care but willing to train the right person.
  • Ability to collaborate within an interdisciplinary setting.
  • Knowledge and understanding of hospice philosophy and principles of death/dying.
  • Valid driver's license and reliable, insured transportation in accordance with New Jersey state requirements.
  • Travel required.

Salary:

  • From $90,000.00
