Job Responsibilities
As a PRN CNA, you will provide compassionate, hands-on care to hospice patients while working closely with our interdisciplinary care team.
- Assist patients with activities of daily living (ADLs), including:
- Bathing
- Dressing
- Grooming
- Toileting
- Feeding
- Assist with patient mobility, transfers, and positioning.
- Monitor and report changes in patient condition to the supervising nurse.
- Provide emotional support and companionship to patients and their families.
- Maintain accurate and timely documentation of all care provided.
- Ensure patient comfort, dignity, and safety while following individualized care plans.
- Adhere to infection control, safety standards, and agency policies.
- Participate in interdisciplinary team meetings and contribute to patient care planning.
Qualifications
- High school diploma or GED required.
- Current Texas Certified Nursing Assistant (CNA) certification required.
- Minimum of one (1) year of experience in hospice, home health, or long-term care preferred.
- Current CPR certification.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a collaborative healthcare team.
- Reliable transportation and a valid driver's license.
