•Manage and coordinate patient panels, including scheduling appointments, follow- ups, and tracking patient engagement.
•Assist in onboarding new patients, ensuring accurate collection and maintenance of medical records.
•Communicate with patients about their health needs, upcoming appointments, and personalized care plans.
•Monitor patient data and clinical outcomes to identify areas for improvement in care delivery.
•Handle administrative duties such as insurance verification, eligibility and patient
registration.
•Collaborate with clinical staff to gather data and verify the accuracy of patient records and documentation.
•Stay informed about changes in HEDIS requirements and healthcare regulations to ensure ongoing compliance.
•Facilitate training and education for staff on HEDIS measures and quality improvement initiatives.
•Assisting members in scheduling appointments and completing preventive health services in accordance with HEDIS/STARS measures.
•Stay up to date on healthcare quality regulations and standards to ensure compliance and adherence to best practices.
•Conduct outreach for patients with chronic conditions to ensure they receive necessary services.
•Contribute to a positive patient experience by providing courteous telephone interactions and appropriately referring cases to clinical staff when necessary.
•Review patient medical charts to verify the provision of necessary services and identify areas for quality improvement.
•Document and notify relevant personnel of any data inconsistencies discovered during patient record reviews.
•Perform all job functions in accordance with federal, state, and local laws, as well as company policies and procedures.

The pay range for this position at commencement of employment is expected to be between $23.00 and $24.00/hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
