We're seeking a skilled and empathetic Full Time Hospice Case Manager to join our dynamic team in the Flint and surrounding areas. This position offers a rewarding opportunity to make a significant impact in the lives of those we serve.

Key Responsibilities:

• Conduct initial and ongoing assessments of patients’ needs to develop comprehensive care plans.
• Provide emotional support and education to patients and families about end-of-life care and self-care techniques.
• Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress.
• Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care.
• Maintain accurate clinical documentation and participate in performance improvement initiatives.

Requirements

• Current Michigan Registered Nurse (RN) License in good standing.
• Minimum of 1 year experience in hospice or palliative care is a plus.
• Ability to work collaboratively with an interdisciplinary care team.
• Empathetic and compassionate approach with patients and families.
• Current driver's license.
• Eligible to work in the United States.
• Able to travel across Flint, Lapeer, and surrounding areas for home visits.
• Willingness to participate in weekend on-call duties on a rotating basis.
• Reliable personal vehicle for home visits across the surrounding areas.
