The Family Support Services Liaison serves as the primary point of contact between families and service providers. This role supports families in accessing resources, navigating systems, and ensuring coordinated, responsive care. The Liaison works collaboratively with internal teams and community partners to promote stability, empowerment, and effective service delivery.
Key Responsibilities:
- Serve as a consistent point of contact for families receiving services
- Conduct intake meetings and ongoing check-ins to assess needs and goals
- Connect families to appropriate community resources and support services
- Coordinate communication between families, providers, and agencies
- Assist families in completing required documentation and applications
- Maintain accurate and timely records in accordance with agency standards
- Monitor service plans and follow up to ensure needs are being met
- Advocate for families within school, healthcare, and social service systems
Qualifications:
- Bachelor’s degree in social work, human services, psychology, or related field preferred
- Experience working with families in community-based or social service settings
- Strong communication, organizational, and problem-solving skills
- Knowledge of local resources in Rockland County a plus
- Ability to maintain confidentiality and professional boundaries
