Up to $80,000 per year

Ridgeway Post Acute is looking for a compassionate and organized Facility Liaison to support and help lead our Social Services Department. If you're passionate about resident-centered care and enjoy building meaningful relationships with families and care teams, this is the role for you.

What You'll Do:

• Support the Social Services Director in coordinating care plans and discharge planning

• Serve as a key point of contact for residents and their families

• Help ensure smooth transitions throughout the care continuum

• Advocate for resident needs and assist with emotional and psychosocial support

• Collaborate with nursing, therapy, and administrative teams to promote positive outcomes

What We’re Looking For:

• Experience in a skilled nursing or healthcare setting preferred

• Strong communication, interpersonal, and problem-solving skills

• Compassionate, empathetic, and able to navigate sensitive conversations

• Organized and able to manage multiple priorities with care and professionalism
