Counselor - Off-Site Facilities

The Counselor–Off-Site Facilities provides direct clinical services to individuals residing in long-term care facilities who seek wellness and mental health supports. The role includes conducting assessments, developing treatment plans, providing individual and group therapy, and maintaining compliance with agency and Medicaid regulations. The counselor will manage a caseload across assigned facilities, collaborating with facility staff and NYCC leadership to ensure high-quality service delivery. Telehealth services may be utilized when appropriate.

Requirements:

• Ability to work independently in off-site settings.
• Valid NYS driver’s license and reliable transportation.
• Strong engagement and communication skills.
• Flexibility in scheduling, including evenings or occasional weekends.

Qualifications:

• Must hold an LMSW, LMHC, or be Limited Permit eligible.
• Experience in Article 31 Mental Health Clinics and/or with aging populations preferred.
• Demonstrated ability to collaborate with diverse groups and maintain professional relationships.
• Commitment to ethical, culturally competent, and compliant clinical practice.
