COTA (Certified Occupational Therapist Assistant) - Sherwood Oaks Post Acute

Provide patient treatment as directed by the Occupational Therapist and per physician’s plan of care.

Document evaluations, daily treatment notes, weekly progress notes, and recertifications per OT Board and payer requirements.

Communicate patient progress, issues, and plans with supervisor and healthcare team members.

Assist in training Restorative Aides and instruct patients, families, and nursing staff on maintenance programs for discharge.

Participate in in-service training programs and discharge planning activities.

Maintain cleanliness and orderliness of treatment areas; report equipment issues promptly.

Ensure compliance with state licensure, continuing education, and professional standards.

Utilize computer programs (e.g., Casamba, Point Click Care) for documentation and data entry.

Perform job duties requiring frequent standing, walking, lifting (up to 100 lbs occasionally), and other physical demands.

Work effectively with patients, families, staff, and professional colleagues in a low-to-moderate noise environment.
