SUMMARY: The Clinical Pharmacist collaborates with primary care providers and staff to improve patient health at Open Door Community Health Centers (ODCHC). This role focuses on integrated medication therapy management to optimize therapeutic outcomes, providing drug information, addressing potential drug therapy issues, and developing productive patient relationships. Responsibilities include patient care, opioid tapering, medication reconciliation, group healthcare visits, and serving as a drug therapy resource for medical personnel. The Clinical Pharmacist supervises pharmacy learners and technicians.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Conduct integrated Medication Therapy Management for patients with chronic conditions and complex medication regimens, including medication adjustments, education, and cost reduction strategies.
• Develop workflows for medication prior authorizations, refills, and reconciliation consistent with ODCHC’s quality standards.
• Integrate pharmacy strategies with ODCHC’s operations, technology, population health, and clinical goals.
• Utilizes motivational interviewing techniques and a trauma informed care approach when interacting with patients and families to help facilitate knowledge transfer and medication adherence.
• Provide education and training on glucometer use, inhaler techniques, lifestyle changes, etc. in individual or group settings.
• Support care teams with post-hospital follow-ups, including medication reconciliation and access services.
• Serve as a drug therapy consultant for providers and healthcare team members.
• Order or perform drug therapy-related assessments and laboratory tests.
• Administer drugs and biologicals by injection as authorized.
• Initiate or adjust drug regimens under prescriber authorization and document changes within 24 hours.
• Assist with opioid tapering and deprescribing to reduce or discontinue unnecessary medications.
• Supervise and mentor pharmacy students, interns, residents and staff as the program expands.
• Coordinate with internal departments and external organizations to enhance patient care and operational efficiency.
• Serve as an active participant in committees focused on improving care and health outcomes of vulnerable populations, such as elders, people with multiple comorbidities, and those with complex social drivers of health.
• Perform other duties as assigned by the supervisor.

• Proficient in writing reports and correspondence.
• Experience with pharmacy technology and willingness to adopt new tools.
• Skilled in interdisciplinary collaboration for patient-centered care.
• Evaluate evidence-based data and information and communicate findings effectively.
• Knowledge of dispensing, monitoring, and counseling practices.
• Expertise in medication therapy, drug interactions, and disease management.
• Strong verbal and written communication, customer service, and computer skills, including proficiency in Microsoft Office and Electronic Health Records.
• Adherence to ODCHC policies and procedures.

EDUCATION AND EXPERIENCE:

• Pharm.D. or BS Pharmacy degree from an accredited college of pharmacy.
• Three to five years’ experience in the practice of pharmacy.
• Maintenance of continuing education requirements.
• Experience with 340B and/or retail pharmacy preferred.

CERTIFICATES, LICENSES, REGISTRATIONS

• Pharmacist license current and in good standing in the State of California.
• Successful completion of state pharmacy board examination.
• Current CPR certification.

SUPERVISORY RESPONSIBILITIES: None.

SUPERVISION AND SUPPORT: Reports directly to the Site Medical Director.
