Job Summary

**This position is in the Navigation Department

This position plays a pivotal role in guiding patients through the complexities of managing health conditions. The role focuses on improving patient adherence to treatment plans, ensuring coordinated care, and enhancing overall health outcomes. Provides personalized education, emotional support, and resources to empower patients in self-managing their health. Collaborating closely with physicians, specialists, and other healthcare providers, this position helps ensure that patients receive comprehensive, effective, and continuous care throughout their health journey.
 

Qualifications

• BS Graduate from an accredited nursing program. Required
• 1-3 years Two (2) years of RN experience in ambulatory care Required
• Additional experience in medical surgical inpatient unit Preferred
• 1-3 years experience in patient relations, education or customer service-oriented position in the healthcare field Required
• Ability to read, write and communicate in English.
• Ability to manage details, people, and multiple projects.
• Demonstrated verbal and written communication skills required.
• Microsoft Word and Epic experience is required.
• Microsoft Excel and PowerPoint experience preferred.
• Registered Nurse New York - New York State Board of Nursing Required
• BLS - American Heart Association Required
• Breast Navigation Certification Preferred
