Care Manager (BH Licensed) - Miami-Dade/Florida Keys

JOB DESCRIPTION Seeking a Florida-licensed behavioral health professional, such as an LCSW, LMSW, APSW, CHES, LPC, LPCC, or LMFT, with experience managing complex behavioral health populations within the Medicaid environment. The ideal candidate will have a strong background in case management and care coordination for members with serious mental illness (SMI) and substance use disorder (SUD), ensuring seamless transitions of care across inpatient, outpatient, and community-based settings. This role requires expertise in multidisciplinary collaboration, member advocacy, crisis intervention, discharge planning, and connecting members to appropriate behavioral health and community resources to improve outcomes and reduce readmissions.
