Job Title: Bilingual RN

Are you an experienced Registered Nurse looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!

Position Summary:

As the Bilingual RN, you would be responsible for supporting SNP (Special Needs Plan) efforts for immediate member outreach to complete Health Risk Assessments and  Care Plans.

Hours for this Position: Monday-Friday (40 hours/week)

More Insight of Daily Responsibilities: 

• Complete HRA questionnaire to provide individuals with an evaluation of their health risks and quality of life
• Oversee patient care plans/unique medical needs
• Provide care to individuals that tailor to their benefits and provider choices

What We Look For:

• Clear and Active RN License in state of FL
• Case Management Experience
• HRA Experience
• SNP Experience
• Bilingual in English/Spanish or Creole
