Employment Type: Full-Time
Compensation: $34–$35 per hour

St. Francis Healthcare Center is seeking a dedicated, organized, and compassionate Admissions Coordinator to join our team on a full-time basis. The Admissions Coordinator plays a key role in supporting the admissions process, coordinating referrals, communicating with residents and families, and working closely with clinical and administrative departments to ensure a smooth transition into our facility.

Key Responsibilities

  • Coordinate and facilitate the admission process for new residents.

  • Review and process referrals, medical records, and required admission documentation.

  • Communicate with hospitals, physicians, case managers, residents, families, and referral sources.

  • Obtain and verify insurance and demographic information.

  • Coordinate admissions with nursing, social services, business office, and other departments.

  • Ensure all required paperwork and documentation are completed accurately and timely.

  • Maintain accurate admission records and referral information.

  • Follow up with referral sources regarding pending and prospective admissions.

  • Assist with coordinating transportation and admission arrangements when needed.

  • Provide residents and families with information regarding facility services, policies, and procedures.

  • Help ensure a welcoming and organized admission experience.

  • Maintain confidentiality and comply with HIPAA and facility policies.

  • Support census development and building relationships with referral partners.

  • Perform other duties as assigned.

Qualifications

  • Previous experience in healthcare admissions, skilled nursing, post-acute care, hospital case management, or a related healthcare setting preferred.

  • Strong verbal and written communication skills.

  • Excellent organizational and time-management abilities.

  • Ability to multitask and prioritize in a fast-paced environment.

  • Strong attention to detail and follow-through.

  • Professional and compassionate approach when interacting with residents and families.

  • Knowledge of insurance verification and healthcare documentation preferred.

  • Proficiency with computers and electronic medical record systems preferred.

  • Ability to work collaboratively with clinical and administrative teams.
