Position Summary
The Acute Care Nurse Practitioner provides advanced inpatient care to abdominal transplant patients as an expert clinician and collaborative partner within the healthcare team. Responsibilities include comprehensive patient assessment, diagnosis, treatment planning, management of acute and chronic conditions, and ongoing evaluation of patient outcomes.
The ideal candidate will have a strong critical care foundation and experience managing medically complex patients.
Responsibilities
Clinical Care
- Perform comprehensive assessments of acute, critically ill, and post-transplant patients
- Diagnose and manage complex medical conditions within the scope of practice
- Develop, implement, and modify evidence-based treatment plans
- Manage changes in patient condition and coordinate appropriate interventions
- Order, interpret, and evaluate diagnostic studies and laboratory results
- Perform approved procedures through the organization's credentialing process
Care Coordination
- Collaborate with transplant surgeons, physicians, nurses, pharmacists, and ancillary staff
- Facilitate patient care transitions within the hospital and across the continuum of care
- Promote health maintenance, disease prevention, and patient education
Education & Leadership
- Serve as a clinical resource for patients, families, nursing staff, and healthcare providers
- Participate in educational initiatives, research, and quality improvement activities
- Mentor and precept students, nurses, and advanced practice providers
Quality & Safety
- Support best practices that enhance patient outcomes and safety
- Participate in quality improvement and performance initiatives
- Maintain compliance with hospital, departmental, and regulatory standards
Qualifications
Required Education
- Master's Degree from an accredited Nurse Practitioner Program- Acute Certification
Required Licensure & Certifications
- Current Louisiana Registered Nurse (RN) License
- Current Louisiana Advanced Practice Registered Nurse (APRN) License - Acute Care Certification
- National Nurse Practitioner Certification
- Basic Life Support (BLS) certification through the American Heart Association
Required Experience
- Acute Care Nurse Practitioner (ACNP) certification
- Previous ICU, Critical Care, or High-Acuity Inpatient experience
- Strong clinical judgment and ability to manage complex patient populations
